The Illinois Supportive Living Program

Located in Chicago, Illinois, Eden Supportive Living gives adults with disabilities a home. Eden Supportive Living belongs to the Illinois Supportive Living Program (SLP), which includes assisted living facilities where people with disabilities can live and receive services.

Established by the Department of Healthcare and Family Services, the SLP is an alternative to nursing home care. People on Medicaid who have disabilities or low incomes qualify for the SLP.

People participating in the SLP can live independently and make decisions for themselves. To secure a spot in an SLP facility, residents must adhere to a few stipulations, which include:

* Undergoing pre-admission screenings. In addition, those who qualify for the Medical Assistance Program must need nursing home care.

* Having a monthly income equal to or more than the minimum amount for Social Security. For 2017, that amount is $735.

* Not participating in other community or home services.
